English and Math Placement Exams
Before you attend a New Student Orientation (NSO), successful completion of these exams is critical. Your scores are used to determine which courses you will register for at NSO.
Change of Major/Add a Minor
You must be a currently enrolled IU South Bend undergraduate student to add or drop a major, minor, and/or certificate using this form. All students at IU South Bend must have at least one major, so students with only one major cannot drop that major without adding one in its place. School Forms
Academic Renewal
Description should read: Current IU South Bend students who want to add or drop a major, minor, or certificate should complete this form. Note: All students must have at least one major, so students with only one major cannot drop that major without adding one in its place.
Requirement to Disclose form
Students in clinical coursework must submit this form at the time of acceptance and then yearly during the remainder of their program.
Clinical Program Admission Decision
Students who received an invitation to join a clinical program after a secondary admission process, must complete this form to inform us if they accept or decline the admission into a specific clinical cohort.
Completing this form allows students to request academic renewal upon return to IU South Bend after they were academically unsuccessful during an earlier attempt at higher education within the Indiana University system.
