Placement Exam Forms
English and Math Placement Exams
Before you attend a New Student Orientation (NSO), successful completion of these exams is critical. Your scores are used to determine which courses you will register for at NSO.
Change of Major/Add a Minor
You must be a currently enrolled IU South Bend undergraduate student to add or drop a major, minor, and/or certificate using this form. All students at IU South Bend must have at least one major, so students with only one major cannot drop that major without adding one in its place. Graduation Forms
College Forms
Completing this form allows students to exercise the grade replacement policy for a maximum of three courses or a maximum of 10 credit hours, whichever comes first.
Completing this form allows students to request academic renewal upon return to IU South Bend after they were academically unsuccessful during an earlier attempt at higher education within the Indiana University system.
