Clinical Application Deadlines

All clinical programs (nursing, dental hygiene, radiography and clinical laboratory sciences) require an application for acceptance into the program. Applications are available through the Advising Center for the BS in Nursing and the AS in Radiography. Applications for the BS in Dental Hygiene are available through the Dental Education program office in Education and Arts room 1250. For information about the application for the BS in Medical Imaging Technology, please contact the department at 574-520-4258. Admissions deadlines are:


Applications are available February 1st with a due date of March 1st for nursing and radiography (AS). Dental hygiene applications are available November 1st with a due date of March 1st. 


Applications are available September 1st and due October 1st for nursing only.

Schedule Adjustments (Withdrawal and Late Withdrawal Appeal Policy)

Students may withdraw from courses online through March 12 (11:59 pm) per the Registrar's policy. Please follow the IU South Bend Academic Calendar for refund dates. Beginning March 13, Dwyer College of Health Sciences students who wish to withdraw from a class must complete a Schedule Adjustment Form (from Registrar's Office or DCHS Advising Center) along with our Dwyer College of Health Sciences Late Withdrawal Appeal Form. Both forms must be signed by the student and faculty of the course, indicating a grade of W or F. In addition, students must submit documentation in support of their late appeal (see form for examples). Completed forms must be submitted to Dwyer College of Health Sciences Advising Center, NS416, and will be reviewed by the Assistant Dean and Dean for approval. All appeal requests must be processed no later than three days prior to the last day of the semester (April 24 at 5 pm). Once reviewed, the student will be contacted for further processing.
